A weekly care coordination checklist for family caregivers
A simple weekly routine can help everyone stay oriented. Use this checklist to agree on a check-in, share responsibilities, keep key information in one place, and prepare for the week ahead.
Choose a weekly check-in time
- Pick a consistent day and time that works for the people who need to take part.
- Decide whether to meet by phone, video, or in person, and keep the check-in focused and manageable.
- Use the same short agenda each week: what changed, what is coming up, and who will follow through.
Assign family tasks clearly
- Write each task as a specific next step, then name one person responsible for coordinating it.
- Agree on a realistic due date and how the person will let everyone know it is done.
- Ask people to confirm their responsibilities so no one is left guessing or duplicating work.
Keep a shared calendar and contact list
- Choose one shared calendar the family can access, and add relevant appointments, visits, errands, and family check-ins.
- Include who is coordinating each event and any practical details the group has agreed to share.
- Keep a contact list with names, roles, and preferred contact details; review it when information changes.
Review next week’s needs
- Look ahead at the calendar and identify commitments that need a ride, reminder, or family point person.
- Check that each open task has an owner and a next step; move or reassign anything that is no longer realistic.
- Write down unanswered questions to discuss with the appropriate person or qualified professional.
